我从2007年10月22日至11月22日进行了专业实践与社会调查活动，为期一个月。主要是对--初中的英语口语教学情况进行了调查。通过调查研究，对在英语口语教学中发现的问题有了初步的了解，为深化教学改革，加强教育教学的针对性，提高教育教学的实效性，探索实施素质教育的新路子，提供了客观依据。

一、调查的具体目标和方法

2. 调查方法

(1).问卷调查

问卷调查在--初级中学进行，共发问卷420份，回收有效问卷415份，接近理想的高效样本数。

(2).座谈会及个案调查

整个调查共召开了两次教师座谈会，四次学生座谈会。这些座谈会，不仅为我们提供了大量生动的典型材料，而且为理性分析提供了基础。

二、调查的内容及结果分析

1.发现问题并明确问题的性质

在口语教学中我发现：（1）部分学生不愿积极参与课堂口语活动；（2）有些学生在口语活动中虽然表现出很高的积极性，但在说英语时却出现了很多语言错误。这种状况与中学英语新课程标准中关于说的教学目标相差甚远。

2.进行调查研究

针对上述问题，我对学生的口语学习和口语活动现状进行了问卷调查。 （调查结果见表1——表3）

调查报告一般由标题和正文两部分组成。

（一）标题。标题可以有两种写法。

一种是规范化的标题格式，即“发文主题”加“文种”，基本格式为“**关于****的调查报告”、“关于****的调查报告”、“****调查”等。另一种是自由式标题，包括陈述式、提问式和正副题结合使用三种。

陈述式如《东北师范大学硕士毕业生就业情况调查》，提问式如《为什么大学毕业生择业倾向沿海和京津地区》，正副标题结合式，正题陈述调查报告的主要结论或提出中心问题，副题标明调查的对象、范围、问题，这实际上类似于“发文主题”加“文种”的规范格式，如《高校发展重在学科建设――****大学学科建设实践思考》等。 作为公文，最好用规范化的标题格式或自由式中正副题结合式标题。

（二）正文。正文一般分前言、主体、结尾三部分。

简报一般都有固定的报头，包括简报的名称、期号、编发单位和发行日期。 1、简报名称 印在简报第一页上方的正中处，为了醒目起见，字号易大，尽可能用套红印刷。

2、期号 位置在简报名称的正下方，一般按年度依次排列期号，有的还可以标出累计的总期号。属于“增刊”的期号，要单独编排，不能与“正刊”期号混编。

3、编发单位，应标明全称，位置在期号的左下方。 4、发行日期，以领导签发日期为准，应标明具体的年、月、日，位置在期号的右下方。

报头部分与标题和正文之间，一般都用一条粗线拦开。 有些简报根据需要，还应标明密级，如“内部参阅”、“秘密”、“机密”、“绝密”等，位置在简报名称的左上方。

报尾部分应包括简报的报、送、发单位。报，指简报呈报的上级单位，送，指简报送往的同级单位或不相隶属的单位，发，指简报发放的下级单位。

如果简报的报、送、发单位是固定的，而又要临时增加发放单位，一般还应注明“本期增发***（单位）”。报尾还应包括本期简报的印刷份数，以便于管理、查对。
