2、《高校毕业生就业协议书》由教育部高校学生司制定，学校招生就业工作处统一翻印，各学院集体到招生就业工作处领取，或者由毕业生持本人学生证到招生就业工作处领取。每位毕业生只有一套《高校毕业生就业协议书》，每套一式四份。

3、任何单位或个人均不得复印、复制、翻印《高校毕业生就业协议书》；在签订《高校毕业生就业协议书》书时，如果《高校毕业生就业协议书》因破损等情况而不能使用时，可持原件到招生就业工作处申请更换；《高校毕业生就业协议书》不得挪用、转借、涂改，否则视为无效。 4、毕业生在协议书上签署个人意见之后，用人单位或学校两方之中只要有一方在协议书上签字，毕业生即不得单方面终止协议的签订工作。

毕业生违约时，必须办理完毕与原签约单位的解约手续，然后将原协议书交还招生就工作处，并换取新的协议书。 5、毕业生如果不慎将协议书遗失，学校原则上不再补发，到毕业派遣时，毕业生回生源地参加二次分配。

若因特殊情况需要补发时，毕业生必须以书面形式提出申请，由所在学院主管毕业生就业工作的负责人签署意见，经招生就业工作处调查并研究之后酌情处理。同时具备以下四个条件时，招生就业工作处方予受理： （1）、经核查，协议书确实属于遗失者； （2）、招生就业工作处收到毕业生的申请书两个星期以上。

（3）、毕业生须交纳相当于违约金数额的费用。 6、凡是通过地方或部委毕业生就业工作主管部门与用人单位签订《高校毕业生就业协议书》的毕业生，签约时可使用他们提供的《毕业生就业协议书》，但是毕业生回校后，必须与学校补签《高校毕业生就业协议书》。

毕业生如果另有选择，则必须与原签约单位解除所签订的协议。 四、《高校毕业生就业协议书》的签订程序。

毕业生与用人单位达成一致后，签约程序是： 1、毕业生认真如实填写基本情况及应聘意见，并签名。 2、用人单位、主管部门及人事调配部门签定意见。

3、用人单位一定将档案详细转递地址填好。 4、各院系签意见。

5、学校就业指导中心签意见。 6、省就业指导中心签证。
